Our client is looking for a care manager to work with adults with learning difficulties in the community, based in Sevenoaks.
The contract is due to start as soon as possible, and is likely to be a 6 - 8 month contract.
You will be required to use a person centred approach to assess, monitor and review the support/personal assistance needs of service users. Work with the service use to develop a package of support/assistance that promotes and maintains independence and well being ; where appropriate involve other agencies and use community resources to provide equality of opportunity, full participation and inclusion in society.
