Complex Care Service

KentSCP have launched a Complex Care Service for both adults and children with long-term health needs. This service is run by Louise Henderson RN, Clinical Case Manager.  KentSCP is a CQC registered, specialist provider of home care, complex care and nursing services.  We currently deliver care and support to clients with a variety of health and social care needs throughout Kent and Surrey, our live-in service covers the whole South East Region. We work with Primary Care Trusts, private hospitals, Case Management companies, Insurance companies, and private individuals.

KentSCP supply and manage Care Support Workers (CSWs) and Registered Nurses with specific skills and training tailored to meet the clinical and complex needs of individual clients, to provide a flexible, person centered home-based care service.

The services we can provide include:-

·Complex Care – both Paediatric and Adult

·Long term health conditions i.e. MS, MND, Parkinsons, Huntingdons

·Palliative care / End of life care 

·Head and Spinal Cord Injury, ABI

·Home Ventilation and Respiratory Support

·24 hour live-in care, Night Service, Respite and holiday Service

·Supporting individuals with complex Learning Disabilities

·Autism & Challenging Behaviour

·Nursing Care

We are very much aware of the increasing number of clients being discharged back into the community who could benefit from receiving an individual care service delivered in the comfort of their own home.  KentSCP offer a bespoke service, unique to each individual.  We manage the clinical care needs of the client, the implementation of specialist, on-site training (such as long term ventilation) and competency sign-off, as well as providing on-going support to our CSWs.

Our Clinical Case Manager is also responsible for the supervision of our CSWs and nurses, to ensure that delegated task competences are continually assessed and reviewed. This provides our clients (the purchaser) with a cost effective solution to training and service costs, and ensures client focused outcomes are consistently achieved.

KentSCP have a Quality Management system ISO 9008:2001 in place, which is continually reviewed and monitored to ensure our policies and procedures are not only adhered to, but are up-to-date and relevant.  We follow stringent recruitment procedures to ensure we only employ the highest quality staff who are compliant with Care Quality Commission (CQC) Standards and are signed off as fully competent to deliver care.

Our initial care needs assessment is free of charge and will be undertaken by our Clinical Case Manager.  At this point, we will be able to provide you with a comprehensive proposal of costs, and a personalised care plan.
